The Expert Patient Programme provides training and support for adults who are living with a long-term health condition. By participating in the programme participants will receive help to manage your condition and meet other people who share similar experiences. The Expert Patient Programme is a six-week course which aims to give you additional skills to self-manage your condition and develop a better relationship with the healthcare professionals who support you.
The courses are delivered by a team of trained volunteers who all have a long-term condition. Each course runs over six weekly sessions which last two and a half hours.
Each course covers a number of topics including:
- dealing with pain, sleep and extreme tiredness
- falls prevention
- appropriate exercise
- relaxation techniques and stress
- healthy eating
- communicating with family, friends and professionals
- planning for the future
